The Three Reporting Options

Each pathway has different registration requirements, measure sets, and eligibility rules. Select the option that best fits your practice type and specialty.

Traditional MIPS
Most flexible • No registration required
  • Registration: None required
  • Measures: Choose 6 from full MIPS inventory, including 1 Outcome or High Priority measure
  • Admin Claims: Automatically evaluated on 4 administrative claims-based measures
  • Categories: All 4 — Quality, Cost, Improvement Activities, Promoting Interoperability
  • Data Completeness: 75% of denominator-eligible cases (performance data must cover ≥75% of eligible patients; small practices receive 3 pts if unmet)
  • PI Reporting: Minimum continuous 180-day period
  • Opt-in / Voluntary: ✓ Available

APP / APP Plus
For MIPS APM participants • No registration required
  • Registration: None required
  • Availability: APM Participants Only
  • APP Measures: 3 required quality measures

  • MSSP ACOs: Must report APP Plus quality measure set
  • Categories: 3 only — Quality, IA, PI (Cost = N/A)
  • IA Credit: Automatically receive full credit in Improvement Activities

Check Your MIPS Eligibility

Determine whether you are required to participate in MIPS, opt-in eligible, or qualify as a voluntary reporter using your NPI and the CMS QPP Eligibility API.

Who Must Participate in MIPS?

CMS determines MIPS eligibility based on your billing activity. Clinicians must meet or exceed all three low-volume thresholds to be required to participate:

    > $90,000 in Medicare Part B allowed charges
  • 2
    > 200 Medicare Part B patients
  • 3
    > 200 covered professional services

Eligibility Categories

Depending on your billing activity and APM participation, you may fall into one of these categories:

  • MIPS Eligible: Must report to avoid negative payment adjustment
  • O
    Opt-In Eligible: Below threshold but can voluntarily choose to participate for a positive adjustment
  • V
    Voluntary Reporter: Not eligible but can submit data for feedback (no payment impact)
  • Q
    QP / Partial QP: In a qualifying APM — may be excluded from MIPS

Which Reporting Option Is Right for You?

Follow these four questions to identify the best reporting pathway for your practice.

Are you a MIPS APM participant?

If you are in a MIPS APM such as an ACO, you may be eligible for the APP or APP Plus pathway.

Is there a relevant MVP for your specialty?

With 27 MVPs available in 2026, many specialties now have a pathway. MVPs are the future of MIPS and will eventually be required.

Do you need opt-in or voluntary access?

Only Traditional MIPS allows opt-in eligible clinicians and voluntary reporters. MVPs and APP do not support these participation types.

Are you a small practice?

Small practices have distinct scoring weights. Quality receives higher weight (40% Traditional/MVP, 75% APP), and Promoting Interoperability is 0%.
